Background: Although rocker shoe decreases plantar pressure in patients with diabetes, its effects on joint contact force of lower limbs is not obvious. Thus, this study aimed to assess the effect of heel rocker on moment and joint contact forces exerted on lower limb joints during walking in patients with diabetes mellitus.

Methods: A total of 10 female subjects with diabetes mellitus and 10 healthy women participated in this study. Musculoskeletal modeling was done by Visual 3D and Open Sim software. For statistical analysis, the independent Student t test was used. The Type I error was set at 0.05.

Results: Hip joint contact force in heel rocker group (1.17 N/BW) was greater than that of the control group (P=0.04). In anteroposterior direction, contact forces of the hip (1. 3 N/BW, P=0.01), knee (1.5 N/BW, P=0.01), and ankle joint (0.47 N/BW, P=0.04) in heel rocker group were greater than those in the control group. But the joint contact force of the heel rocker group in mediolateral direction was less than that in the control group (1.31 and 0.15 N/BW in hip and knee joints, respectively, P=0.01).

Conclusion: Rocker shoe decreased vertical joint contact force in the ankle joint and increased the vertical and anteroposterior joint contact forces in proximal joints (knee and hip). Thus, it may lead to hyperactivity and pathology in these joints. In this regard, prescription of heel rocker shoes for patients with diabetic mellitus must be done with caution.
